How Organizations Collapse Into Firefighting

Every organization runs on 4 levels of work.


Technical. Tactical. Operational. Strategic.


Technical - does the work in accordance with process. 

Tactical - ensures the work is done in accordance with process. 

Operational - writes the process. 

Strategic - wields the entire organization.


Each level has the same 3 core systems: 

Daily operations - do what needs to be done (planning and execution)  

Process improvement - learn from feedback, improve the standard.

Accountability - make sure the level below you is doing the whole job.


Simple enough. So why do most organizations collapse into chaos?


Because accountability is the foundation and Ashby's Law quietly destroys it.


Your processes generate more variety than your leaders can absorb. They can't check every standard. They don't know enough. They can't see enough.


Which is why accountability becomes selective. Which feels and seems unfair. Then it becomes ceremonial. Then it's gone.


Here's what happens next, in order:


CI dies first.


Why write better procedures for a team that won't follow the ones you have?


Improvement becomes theater. Leaders stop investing because the returns are zero.


Then operations devolves into firefighting.


Without process improvement and without accountability, issues don't get closed, they recur.

 

It's the same problems, week after week.  Operators complain about "no accountability."


Then leaders collapse to the tactical level, the primary functions of the strategic and operational levels are essentially gone.


The plant manager is on the floor. The ops manager is supervising shifts.

 

Every leader is running toward fires with a bucket.


And the supervisors? Senior leaders are doing their jobs for them; which means nobody's doing the senior leader's job.  And the supervisors get stepped on so much they disengage.


Nobody is building the system that stops the fires from starting.


This is the org that's always busy and never improving.


The fix isn't more discipline. It's not a better CI program. It's not even better people.


It's building an accountability infrastructure sophisticated enough to keep pace with the complexity your operation generates.
